Begin your child's evaluation.

Your online intake takes about fifteen minutes. After payment, you'll pick a first-visit time that works for your family. Most first visits with Dr. Levine are scheduled within one to two weeks of completing intake.

Step 01

Complete the pre-visit questionnaire

A structured intake covering your child's developmental history, your observations, and the questions you're hoping the evaluation will answer. Most parents finish in ten to fifteen minutes.

Step 02

Pay the $1,499 evaluation fee

Secure checkout. HSA and FSA eligible. Affirm payment plans available with 0% APR options for qualifying families.

Step 03

Book your first visit

Pick a time that works for your family. Most intake visits with Dr. Levine are scheduled within one to two weeks of booking.

Eligibility

First, where are you located?

We're currently licensed in New York and Texas. Telehealth visits are only available to patients physically located in licensed states at the time of each visit.

Your information stays private and is used only to facilitate your child's care. We never share parent or patient information with third parties.
